Arrival & Departure Record - CBP Form I-94W

If you visit the United States under the Visa Wavier Program, you will need to complete the Customs & Border Protection Form I-94W. It's the green form.

Questions related to inadmissibility issues are part of the I-94W. If you answered "yes" to any questions on the reverse side of the I-94W, contact the United States Embassy or Consulate in your country to obtain a visa prior to travel.

You will need to sign and date your I-94W. That indicates that you have agreed to waive your right to a hearing before an immigration judge if you have been found inadmissible.
